Description
 
Qualified Support Professional (QSP)
 
This position is responsible for the development of comprehensive Individual Service Plans (ISP), coordination of services, overseeing the implementation of ISPs, and providing the general case management to ensure the individuals with intellectual and physical disabilities in services attain personal goals.
 
Supervision
 

  • Is supervised by the Social Services Supervisor and, in the absence of the Supervisor, reports directly to the Assistant Director of Social Service

1.         Evaluate consumers, establish program and service recommendations and follow through on implementation of plans.
2.         Must be able to interpret licensing and regulatory standards and ensure daily performance             co insistent with requirements.
3.         Must be able to work with minimal supervision.
4.         Must be able to work flexible hours when required.

Qualifications
 
1.         Must be 21 years of age.
2.         Must possess a valid Illinois Driver’s License.
3.         Must be a DHS certified QSP.
4.         Bachelor’s Degree in Human Services field, including, but not limited to, sociology,
            special education, rehabilitation counseling or psychology.
5.         One years of experience with Developmental Disabled Required.
 
Other
 
1.         Must pass a criminal background check.
2.         St. Coletta’s of Illinois is a drug and alcohol free environment.  Employees must pass a
            drug/alcohol screening test.
3.         Must obtain required CEUs to retain certifications.
